It depends on the substance: 0.5 ml for water at 1,000 mg/ml, but 10 ml for a syrup at 50 mg/ml. There is no universal answer, because milligrams measure mass and millilitres measure volume.
Millilitres = milligrams ÷ concentration, and milligrams = millilitres × concentration. The concentration in mg per ml is the only thing linking the two units.
Only for water, and near enough for dilute watery solutions. One millilitre of water weighs one gram — that is how the gram was originally defined. Oil is lighter and honey heavier, so treating everything as water is the commonest mistake here.
Divide it down to one millilitre: 250 ÷ 5 = 50 mg/ml. Percentages work too — a 1 % solution is 10 mg/ml, because a percentage means grams per 100 ml.
